EFFICACY AND SAFETY OF STEREOTACTIC BODY RADIATION THERAPY IN OLIGOMETASTATIC PROSTATE CANCER: A SINGLE-ARM META-ANALYSIS OF PROSPECTIVE STUDIES

Introdução e Objetivo

Stereotactic body radiotherapy might delay androgen deprivation therapy and promote satisfactory disease progression-free survival in patients with oligometastatic prostate cancer, but meta-analysis addressing these endpoints is lacking. We aimed to quantitatively analyze prospective data on stereotactic ablative radiation as the metastasis-directed therapy for node and bone oligometastases of prostate cancer.

Método

We systematically reviewed follow-up studies published until April 16, 2023. MEDLINE and Cochrane Central were searched, according to Preferred Reported Items for Systematic Reviews and Meta-analysis guidelines. Data were pooled in a single-arm meta-analysis using a random-effects model. The risk of bias in included studies was assessed through Cochrane’s recommended tools.

Resultados

Seven prospective studies, with 478 patients were included. At 2 years, pooled progression-free survival was 41,2% (95% CI: 26,3-56,0%; I^2=72,45%, p=0,027), local progression-free survival, 96,5% (95% CI: 93,3-99,7%; I^2=56,98%, p=0,054) and androgen deprivation therapy-free survival, 57,2% (95% CI: 48,1-66,3%; I^2=0%, p=0,955). The prevalence of late grade III toxicity was 0,4% (95% CI: -0,2-0,01%; I^2=0%, p=0,955), in a median follow-up of 26,45 months.

Conclusão

In a population with oligometastatic prostate cancer, stereotactic body radiotherapy was well tolerated and encouraging for patients concerned with systemic therapy´s side effects. Long-term studies are needed to determine whether metastasis-directed therapy advantages are observable in overall survival.
